The poison was almost certainly on the hands of the assassin, as the radiation could not be absorbed through the skin. When the victim went to the bathroom, or looked the other way - the assailant most-likely dipped their fingers (with the radioactivity on them) into the drink. When it was ingested, it poisoned him.
They found a radiation trail all over London.
